J1 Alien Physician Visa Application and Change of Visa Category from the US for Indian IMGs -


Documents required -

  1. Cover Letter 
  2. SON Application
  3. Miscellaneous application form with photograph.
  4. Notarized SON application – 2 copies.
  5. Notarized Passport copy.
  6. Notarized USA Visa copy.
  7. Notarized I-94 copy.
  8. Notarized Forms (Number of DS-2019 forms) DS-2019 copies.
  9. Notarized lease agreement copy – proof of present address in US.
  10. Notarized Offer letter copy.
    • Offer letter from your program including your income and duration of the program.
  11. Money order for $34. 
    • You can get the money order made at your bank or at USPS.
  12. Prepaid return mailing envelope.
